SCHOLARSHIP APLICATION

SCHOOL YEAR: 2009-2010

AWARD

The Alamo Mission Chapter Scholarship is to be awarded by the Alamo Mission Chapter, DRT, to one high school senior who has demonstrated outstanding scholarship and leadership. The recipient will enroll as an undergraduate in a Texas college or university in the fall semester following high school graduation. The award will be a maximum of $4,000.00 ($500.00 per semester for four years.)

ELIGIBILITY

1. SCHOLARSHIP: Applicants must have proven scholastic ability and must have ranked in the upper 10 percent of their class at the end of their junior year in high school.

2. LEADERSHIP: Applicants must have demonstrated leadership ability in school, community or part-time and/or summer work.

3. HIGH SCHOOL GRADUATION: Applicants must be graduating seniors in May/June of the current school from an accredited high school in the San Antonio area.

4. COLLEGE ADMISSION: Applicants must meet all requirements for admission to a college or university lo-cated in the state of Texas for the purpose of undergraduate work in the fall semester following high school graduation.

5. APPLICATION FOLDER: Applicants must prepare an application folder as prescribed in the following instruc-tions and submit it directly to the DRT Scholarship committee for evaluation.

FORMAT FOR APPLICATION FOLDER

Please read carefully: All of the following information and documentation is required. The information must be typed, arranged by section (I-V) in the prescribed sequence, and fastened in a plan pocket folder.

Section I—Personal

1. Full Name

2. Social Security Number

3. Birth (Including month, date, year, city, state, country)

4. Permanent Address (include street, city, state, zip code)

5. Telephone Number and E-mail Address

6. Names of Parents (give address if different from #4)

7. Name, Address and Telephone Number of High School Attending

8. Projected Graduation Date from High School

9. Name and Location of Intended College or University

Section II—General

1. List in the order of importance to you no more than five awards or honors received during high school.

2. List volunteer participation in community, charitable or religious activities and/or organizations (no more than five) during the past two years.

3. Enclose a letter or recommendation from a person in your school or community, other than a relative, describing your character.
